Mixed endometrial stromal and smooth muscle tumour Kirjallisuudessa on kuvattu: Stroomasarkoomien sekä strooma/sileälihas sekatuumoreiden metastaaseja, jotka eivät muistuta primäärikasvainta (esim. metastaasissa vain toinen sekatuumorin komponenteista tai jopa solukomponentteja joita ei primäärituumorissa ole nähtävissä)

Strooma- vs sileälihasmarkkerit, IHC:n tulkintaan liittyviä ongelmia Stroomasolukko voi ilmentää sileälihasmarkkereita (>>SMA, >desmin; h-caldesmon?) Toisaalta osassa sileälihastuumoreita lihasmarkkerit voivat olla jopa negatiivisia (>leiomyosarkoomat, epiteloidit tuumorit) CD10 ei ole luotettava stroomasolun markkeri, myös sileälihastuumorit voivat ilmentää (>hypersellulaarinen leiomyooma, myös epiteloidi) IHC paneelin oltava riittävän laaja IHC tulkittava aina suhteessa solujen morfologiaan Kokonaismorfologia (kasvutapa, verisuonipattern jne) huomioitava
YHTEENVETO: Kohdun mesenkymaalisissa kasvaimissa suurta morfologista variaatiota (histologiset variantit ja sekatuumorit) Erotusdiagnostiikka tärkeä, maligniteetin kriteerit sileälihas- ja stroomatuumoreille aivan erilaiset! Primäärituumori ja metastaasi eivät välttämättä varsinkaan strooma/sekatuumoreissa muistuta toisiaan Riittävä sampling perusedellytys sekä histotyypin määrittämistä että maligniteetin arviomista ajatellen IHC tulkinta: riittävän laaja paneeli (CD10 ja useampi lihasmarkkeri), aina suhteutettava morfologiaan
