Mielihyvä ja palkitsevuusalueet aivoissa: The mesolimbic motivational and rewarding system Cocaine DA Alcohol NAC Heroin Nicotine
DOPAMINE is the main transporter in NAC
Palkistsemisjärjestelmät: reward pathway (RP) in the brain The reward system is part of a motivational system in our brain which guides us in our lives, causing us to seek things that we need (or are necessary) while avoiding those which might be dangerous Any pleasurable action activates RP Endogenous opiates modulate the actions of RP via dopaminergic system Chronic alcohol causes a dysfunction of this system
Opioid binding in Addiction and Dependence areas
Roth-Deri et al 2008 Merrer et al 2009 The mesolimbic motivational and rewarding systems is under the influence of the endogenous opioid system. Expression of the opioid system in the brain Endorphin-producing neurons are located primarily in the arcuate nucleus, whereas enkephalins and dynorphins producing neurons are widely distributed throughout the brain. Opioid receptor expression (rat) Opioid peptide (ligand) distribution (rat)
Aivojen endokannabinoidisysteemi näyttelee tärkeää osaa alkoholin (ja opiaattien) vaikutuksien välittymisessä ja riippuvuuden keittymisessä Alkoholinkäyttö lisää endogeenisten opiaattien vapautumista (β-endorfiinit), jotka välittävät alkoholin miellyttävää ja vahvistavaa vaikutusta Embellished from Gianoulakis 1998 Opiaattiantagonistit estävät (endogeenisten) opiaattien vaikutusta vähentävät alkoholin-himoa ja vähentävät retkahdusvaaraa Naltreksoni ja nalmefeeni tehokkaita alkoholiongelman hoidossa
Acute alcohol consumption induces β-endorphin release in the human β -endorphin release in response to acohol (human) brain β-endorphin induces euphoria and pleasure, which contributes to the positive reinforcing effects of alcohol consumption µ opioid receptor knock out mice will not self administer alcohol (Job et al 2007, Roberts et al 2000) β-endorphin indirectly stimulates mesolimbic dopamine release (Heilig 2011)
Opioid system Opioids The opioid system consists of three different subtype receptors and three different endogenous opioid peptides: Opioid Synapse β-endorphin or Dynorphin All opioid subtype receptors are inhibitory G-coupled receptors. µ opioid receptor agonist (β-endorphin) binding reduces excitability at the post-synapse and consequently reduces transmitter release Opioid receptor antagonists nalmefene and naltrexone blocks the actions of endogenous and other opioids Kieffer and Evans 2009 Opioid Receptors µ κ Opioid Molecule
Aivot ja riippuvuus = käyttäytymistä säätelevän motivoivan systeemin säätelyhäiriö The "go" system is "running off on its own" instead of interacting with the "stop" system. The behavioral repertoire has narrowed such that alcohol becomes the focus of the whole life The natural reward/motivational systems have been hijacked - alcohol is driving the system The mesocorticolimbic system is sensitised to alcohol and alcohol associated cues
The reward pathway (RWP) in the brain The reward system is part of a motivational system in our brain which guides us in our everyday lives, causing us to seek things that we need (or are necessary) for the survival - Drugs can hijack the RWP and can OVERdrive the system - Any pleasurable action activates RWP - Behavioral addiction
Pharmacological tools to influence the actions of these systems How does opioid antagonists nalmefene and naltrexone help the patient to reduce alcohol consumption?
Termistöä Nalmefeeni on opioidinsalpaaja Opiaatti Oopiumperäinen morfiinin kaltaiset luonnon (morfiini) tai puolisynteettinen (tamaloli) opioidi Opioidi, kattotermi, edelliset + elimistön sisäsyntyisiä morfiinin tavoin vaikuttavat aineet (endorfiinit: dnorfiinit, enkefaliinit)
Binding of [ 11 C]-carfentanil (morphine) before after nalmefene (20 mg, 3 h) Maximum µ- occupancy reached after 3 h - 94-100% Decline in receptor occupancy slower than decline in plasma conc. - 83-100% 26 h Neuropsychopharmacology, 2005, 30, 2245-2253 after
Conclusion: How does opioid antagonists help the patient to reduce alcohol consumption? They reduce the reinforcing effects of alcohol by modulating the corticomesolimbic system which is dysregulated in alcohol dependent patients NALMEFENE: Modulation of the kappa/dynorphin system may help to decrease negative reinforcement in alcohol dependence
Päihteetriippuvuus ja aivot Päihderiippuvuus on krooninen aivosairaus, jossa on muutoksia mm. palkitsevan aivoradan toiminnassa, synapsien rakenteessa ja toiminnassa sekä muutoksia dopamiiniaineenvaihdunnassa Muutokset aiheuttavat käyttäytymistä, jossa yksilö on menettänyt sisäisen kontrollin ja toimii tahdostaan riippumatta terveytensä vaarantaen Käyttäytyminen on palkitsevaa, itseään vahvistavaa
