Onnettomuus 19.7.1989 15:16 DC-10-10 (N1819) United Airlines (232) koki massiivisen moottorivaurion kesken lennon Lentokoneessa kuultiin kova pamaus, jonka jälkeen hydrauliikkapaineet menetettiin ja lentokone tuli ohjailukyvyttömäksi Miehistö yritti pakkolaskua Sioux Gateway-lentokentälle Pakkolaskun yhteydessä kone iskeytyi maahan, pyöri ympäri ja syttyi tuleen 296 matkustajasta tai henkilökunnasta 111 menehtyi törmäyksessä tai sitä seuranneessa tulipalossa
Vaurioanalyysi Myöhemmässä vaurioselvityksessä kävi ilmi, että moottorivaurio aiheutui turbiinikiekon murtumisesta (fan disk) Murtuminen aiheutti moottorikappaleiden leviämisen ympäristöön (kappaleiden energia oli niin suuri, ettei moottorin suojakuori riitänyt estämään niiden leviämistä) Kappaleet vaurioittivat hydraulikoneistoja ympäristössä ja aiheuttivat hydraulipaineen menetyksen Hajonnut kiekko löydettiin myöhemmin maissipellolta Iowasta ja vaurion alku voitiin tutkia tarkemmin.
Miksi kiekko murtui?... ja millä voidaan estää vastaisuudessa?
Ensisiijainen murtuma oli säteen suuntainen "boreto-rim" murtuma Vaurio alkoi keskiakselin reiässä olevasta väsymisvauriosta Loppumurtuma seurasi kriittisen kokoon kasvaneesta väsymisvauriosta
Vaurioanalyysi Metallografia osoitti että väsymisvaurio oli alkanut "nitrogen-stabilized type I hard alpha defect" (hauras epämetallisulkeuma) Sulkeuma oli syntynyt valmistuksen yhteydessä Sulkeumaa ei havaittu valmistuksenaikaisessa ultraäänitutkimuksessa, makrosyövytyksessä ja tunkeumanestetarkastuksessa.
Loppumurtuman hetkellä särö oli kriittisessa mitassa normaaleissa käytönaikaisessa kuormituksessa Väsymisjuovista päätelty syklimäärä oli lähellä moottorin nousu/laskusyklien määrää (15503) Särö oli ydintynyt hyvin varhain Ilmeisesti hauras sulkeuma murtui ensimmäisen täyden tehon käytön yhteydessä ja ydinsi väsymisvaurion.
760 nousu-lasku sykliä ennen onnettomuutta moottorille tehtiin tarkastus Tällöin, särö oli 0.476" (n. 12 mm) ja jäi havaitsematta

Separat ion of the t i tanium al loy stage 1 fan rotor disk was the resul t of a fat igue crack that ini t iated from a type 1 hard alpha metal lurgical defect on the surface of the disk bore. The hard alpha metal lurgical defect was formed in the t i tan ium a l loy mater ia l dur ing manufacture of the ingot from wh ich the disk was forged. The hard alpha metal lurgical defect was not detected by ul t rasonic and macroetch inspect ions performed by General Elect r ic Ai rcraf t Engines dur ing the manufactur ing process of the disk. The meta l lurgica l f law that formed dur ing in i t ia l manufacture of the t i tan ium a l loy wou ld have been apparent i f the par t had been macroetch inspected in i ts f ina l par t shape. The cavi ty associated wi th the hard a lpha meta l lurgica l defect was created dur ing the f ina l mach in ing and/ or shot peen ing at the t i me of GEAE ' s manufacture of the disk, af ter GEAE ' s u l t rason ic and macroetch manufactur ing inspect ions. 19. A detectable fatigue crack about 0.5 inch long at the surface of the stage 1 fan disk bore of the No. 2 engine existed at the time of the most recent United Airlines inspection in April 1988 but was not detected before the accident. 20. The discoloration noted on the surface of the fatigue crack was created during the FPI process performed by UAL 760 cycles prior to the accident, and the discolored area marks the size of the crack at the time of this inspection. 21. The inspection parameters established in the United Airlines maintenance program, the United Airlines Engineering Inspection Document, and the General Electric Aircraft Engines shop manual inspection procedures, if properly followed at the maintenancefacility, are adequate to identify unserviceable rotating parts prior to an in-service failure.
Probable Cause The National Transportation Safety Board determines that the probable cause of this accident was the inadequate consideration given to human factors limitations in the inspection and quality control procedures used by United Airlines' engine overhaul facility which resulted in the failure to detect a fatigue crack originating from a previously undetected metallurgical defect located in a critical area of the stage 1 fan disk that was manufactured by General Electric Aircraft Engines. The subsequent catastrophic disintegration of the disk resulted in the liberation of debris in a pattern of distribution and with energy levels that exceeded the level of protection provided by design features of the hydraulic systems that operate the DC-lo's flight controls.

Such research should encourage the development and implementation of redundant ("second set of eyes") inspection oversight for critical part inspections, such as for engine rotating components.
