Kehitysvammaisuus ICD-10 Älyllinen kehitysvammaisuus on tila, jossa mielen kehitys on pysähtynyt tai epätäydellinen. Heikosti kehittyneitä ovat erityisesti kehitysiässä ilmaantuvat taidot. Näitä ovat yleiseen älykkyystasoon vaikuttavat kognitiiviset, kielelliset, motoriset ja sosiaaliset kyvyt. Älylliseen kehitysvammaisuus saattaa esiintyä yksinään tai siihen voi liittyä mitä tahansa muita henkisiä tai ruumiillisia häiriöitä. Älyllisesti kehitysvammaiset voivat sairastua mihin tahansa mielenterveyshäiriöön.

Valitun testin tulisi soveltua testattavan toimintatasoon ja mahdollisiin lisävammoihin, esimerkiksi puheilmaisun vaikeuksiin, kuulon heikkouteen ja fyysisiin vammoihin. Mikäli suinkin mahdollista, pitäisi haastattelemalla testattavan vanhempaa tai jokapäiväisen elämän taidot tuntevaa huoltajaa täydentää sosiaalisen kypsymisen ja sopeutumisen tason määrittelyä. Jos paikalliseen kulttuuriin vakioituja menetelmiä ei käytetä, diagnoosia tulee pitää vain alustavana arviona.

Autismi ICD-10 Laaja-alaisille kehityshäiriöille ovat luonteenomaisia laadulliset poikkeavuudet molemminpuolisessa sosiaalisessa vuorovaikutuksessa ja viestinnässä sekä kapea- alaiset, kaavamaiset harrastukset ja toiminnot. Nämä laadulliset poikkeavuudet ovat kaikissa tilanteissa vallitseva toimintapiirre, vaikka ne voivat asteeltaan vaihdella. Useimmiten kehitys on poikkeavaa varhaisesta lapsuudesta asti ja tavallisesti häiriö ilmenee ennen 6 vuoden ikää. Jonkinasteista yleistä kognitiivisten toimintojen heikkenemistä esiintyy usein, muttei aina. Häiriöt määritellään älykkyysikään nähden poikkeavan käytöksen perusteella. Häiriöiden alajaottelu ei ole täysin vakiintunut. Data from Sweden on 241 children with CP indicate that 36% were born at a gestational age (GA) of less than 28 weeks; 25% at 28 to 32 weeks GA; 2.5% at 32 to 38 weeks GA; and 37% at term. There is agreement that CP is due to a defect or lesion in the developing brain, which may have had its onset in the prenatal, perinatal, or postnatal period. Autismin genetiikka Autismin kaksos- ja perhetutkimukset tukevat perimän osuutta *(arvioidaan usein 70 80 %:ksi). *Geneettinen tausta on kuitenkin heterogeeninen tavallisimmatkaan perinnölliset muodot eivät edusta kuin 1 2 %:a. -Taustalla on tunnettuja kromosomipoikkeavuusoireyhtymiä (n. 10 %), (useimmiten uniikkeja) ja myös de novo -geenien kopiolukumäärämuutoksia (n. 10 %, osuus on lisääntymässä) sekä kytkentäanalyysien mukaan eri lokuksiin paikantuvia ja autismiin assosioituvia geenejä (10 20 %) (Geschwind 2008, 2011). *Vallitseva geneettinen malli viittaa polygeeniseen periytymiseen. *Epigeneettiset tekijät ovat myös tärkeitä. -Esimerkiksi Jamesin ym. (2010) tutkimuksessa polymorfismi äidin folaattiaineenvaihduntaan liittyvässä geenissä lisäsi lapsen autismiriskiä. Näiden äitien DNA oli hypometyloitunut, mikä vaikutti kohdunsisäiseen ympäristöön tärkeiden kehitysvaiheiden aikana.

Psykologista taustaa erityisopetuksen tarpeelle Ek y.m. 2004 * Taustaa: kokemuksemme mukaan lapset, joilla on täsmällinen diagnoosi saavat helpommin apua koulussa kuin lapset, joilla on vastaavia ongelmia ilman spesifiä diagnoosia. * Aineisto: Tukholman läänin yhden kunnan kaikki 4. luokkalaiset (N=591), joiden vanhemmille ja opettajille koulutarkastuksen yhteydessä annettiin täytettäväksi kyselylomakkeet (Conners, toiminnan ohjaus (EFSS)). PhD; Risch, Neil PhD To our knowledge, this study is the largest population-based twin study of autism that used contemporary standards for the diagnosis of autism. All twin individuals underwent a thorough diagnostic and cognitive examination that included a structured interview and observation that allowed differentiation between autism and other delays of development.13 14 The results suggest that environmental factors common to twins explain about 55% of the liability to autism. Although genetic factors also play an important role, they are of substantially lower magnitude than estimates from prior twin studies of autism. Nearly identical estimates emerged for ASD, suggesting that ASD presents the same liability spectrum as strict autism.

OPD based on parent-report is associated with gross motor skills in children with cerebral palsy (CP), however this underestimates prevalence. Almost all children with severe CP have dysphagia; little is known about mild CP. WHAT THIS STUDY ADDS: The prevalence of directly assessed OPD in preschool children with CP is 85% (70% in GMFCS I; 100% in GMFCS V). OPD was prevalent even in mild CP. To investigate whether drooling in children with cerebral palsy (CP) in general and in CP subtypes is due to hypersalivation. METHOD: Saliva was collected from 61 healthy children (30 males, mean age 9y 5mo [SD 11mo]; 31 females, mean age 9y 6mo [1y 2mo]) and 100 children with CP who drooled (57 males, mean age 9y 5mo [3y 11mo], range 3-19y; 43 females, mean age 10y 1mo [4y 9mo], range 4-19y), of whom 53 had spastic, 42 had dyskinetic, and five had ataxic CP. Almost all children were affected bilaterally, and 90 of them were at Gross Motor Function Classification System levels III or higher. The saliva was collected by the swab saliva collection method. The intensity of drooling was evaluated using the drooling quotient. RESULTS: No difference was found in the flow rates, age, or sex between healthy children and children with CP who drooled. On additional subgroup analysis, the flow rates of children with dyskinetic CP differed statistically from those of healthy children (submandibular p=0.047, parotid p=0.040). INTERPRETATION: This study supports the finding in previous studies that no hypersalivation exists in children with CP who drool. Dysfunctional oral motor control seems to be responsible for saliva overflow from the mouth, whereas increased unstimulated salivary flow may occur in children with dyskinetic CP as a result of hyperkinetic oral movements.

Populations of children with neurological impairment who commonly experience dysphagia include, but are not limited to, those with acquired brain impairment (for example, cerebral palsy, traumatic brain injury, stroke), genetic syndromes (for example, Down syndrome, Rett syndrome) and degenerative conditions (for example, myotonic dystrophy). Objectives: To examine the effectiveness of interventions for oropharyngeal dysphagia in children with neurological impairment. Three studies met the inclusion criteria for the review. Two studies were based on oral sensorimotor interventions for participants with cerebral palsy compared to standard care and a third study trialled lip strengthening exercises for children with myotonic dystrophy type 1 compared to no treatment The review demonstrates that there is currently insufficient highquality evidence from randomised controlled trials or quasirandomised controlled trials to provide conclusive results about the effectiveness of any particular type of oral-motor therapy for children with neurological impairment.
